1. What kind of things can HLC help me with?

  If you need someone to help you make sense of all the different diets out there, different exercise regimens, advice about
  supplements, we can help.

  If you know what you need to do to improve your health, but need someone to help you tackle a big project, help keep you motivated,
  and provide an accountability partner, we can help.

  If your doctor wants you to lose weight, quit smoking, start exercising and you need help to make it happen, we can help.

  If you have made and broken New Years Resolutions year after year, but really want to succeed, we can help.

  If you have aches and pains but want to be more active, we can help.

2. How do I get an appointment? Should I get a doctor’s referral?

  • Physicians may refer patients to our coach, Susan Boyko RN, BSN, who will work directly with the referring physician to achieve
  mutually agreed upon goals
  • Physicians may refer patients who wish to work on lifestyle changes to see Dr. Huebner or Jane Denay CFNP for an initial
  musculoskeletal evaluation and goal-setting session, followed by coaching on subsequent visits.
  • Patients may self-refer to work directly with our certified health coach. Susan Boyko will help develop appropriate goals and work
  with individuals to implement them
  • Patients may self-refer for an initial musculoskeletal evaluation and goal-setting session with Dr. Huebner or Jane Denay,
  CFNP, followed by coaching on subsequent visits

3. Does insurance cover this?

  Fees for initial visits with Dr. Huebner or Jane Denay CFNP can be filed to insurance. Your insurance may or may not cover this
  appointment. If your insurance denies or if the you do not have insurance, the fee is $300. Coaching visits will be $40 per ½
  hour and are not billable to insurance.

4. What is the relationship between Harbor Arthritis Center and Harbor LifeStyle Center?

  Harbor Arthritis Center is the medical practice of Dr. Charles Huebner, a board certified Rheumatologist and Internist and Jane
  Denay, CFNP, a Nurse Practitioner with years of experience in pain management and Rheumatology.

  Harbor LifeStyle Center is a division of Harbor Arthritis Center. Dr. Huebner has a passion for preventive medicine, healthy eating
  and those lifestyle strategies that increase health rather than just treating illness. Harbor LifeStyle Center is the natural result of
  these interests.

5. When your brochure states that you will work with other medical professionals, what do you mean?

  We will work with your primary care physician to help you reach your goals. There may also be times when a referral to a Physical
  Therapist, Diabetic Educator, personal trainer, counselor or dietician may be appropriate. These will always be discussed with you
  and your doctor.
